Comments:
An unannounced renewal inspection was conducted on June 6, 2023 from approximately 11:50 am to 1:00 pm. There were 6 children in attendance. 1 staff record, and 6 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected.

Interviews were held with the provider throughout the inspection. The designated areas for childcare were deemed appropriate and are equipped with age-appropriate toys to meet the needs of the children. Children were observed watching television and reading while the licensing inspector was at the home.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 06/12/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description: Based on records review, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance or within 30 days after the first day of attendance. Evidence: The record for child #6, date of enrollment 08/18/22, did not contain evidence of a physical examination.

Description: Based on observation, the provider failed to ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. Evidence: The maximum licensed capacity of the home is 5 children. There were 6 children in attendance.

Description: Based on interview, the provider failed to keep a written record of children in attendance each day. Evidence: When asked to see the written record of attendance the provider stated that she did not have a written record of attendance.

Description: Based on observation, and interview, the center failed to ensure that each child who is two years of age and older shall have access to an individual location in which to keep clothing, toys, and belongings. Evidence: There were two bookbags on the floor in the living room. There were 4 children ages 2 to 4 years in attendance. When asked where the children put their personal belongings such as bookbags, clothing, toys, and other personal belongings, the provider stated that she did not have additional storage for those items.

Description: Based on observation, the provider failed to ensure that playpens shall not be occupied by more than one child and shall not contain any pillows or filled comforters. Evidence: There was a playpen that was occupied by two children, age 19 months. The playpen had a large filled comforter and pillows in it .
