Technical Assistance:
Reviewed household member guidelines to include background checks and TB screening. Reviewed transportation guidelines to include use of car seats, insurance, and inspection requirements. Reviewed guidelines for smoking to include never smoking in the house when children are present.
Comments:
A complaint inspection was initiated on September 7, 2023 in response to a complaint that was received on August 29, 2023 in reference to supervision, transportation, diaper changing procedures, smoking, and household members. Documentation was reviewed and interviews were conducted. The preponderance of evidence gathered during the investigation does not support violation of standards regarding the complaint.
Violations were cited that were not related to the complaint.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice sign, and return it to me within 5 business days from today, 09/20/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Based on record review, the provider failed to maintain a complete child record. Evidence: The record for child #1, date of enrollment 08/28/2023, did not contain the name, address and phone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Child #1 was in attendance.
