Comments:
An unannounced monitoring inspection was conducted on 6/1/23. The provider has one child in care totaling 4 points and other 6 children attends after school. The provider has lots of toys for the children to play with. Children bring food from home. Provider is playing with the child in care. A violation notice was given to the provider with this summary. Based on the review of three children's records, the provider failed to get some of the information to make the file complete. Evidence: Three children's records reviewed.1 child is missing one emergency contact and 2 are missing 2 emergency contacts.

I will ask the parents for the emergency contacts.
