Comments:
An unannounced monitoring inspection was conducted on 07/14/2023 to follow-up on previous areas of non-compliance. There were 24 children, ages 2 months-5 years, and 11 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, programs, special care provisions and emergencies, and nutrition. The children were observed during free choice activities, and having breakfast; infants were observed in play and sleeping and eating on demand. A total of 5 children?s records, 5 staff records and 1 medication were reviewed. The inspector arrived for the inspection at 8:45 am and departed at 12:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the provider.
REPEAT VIOLATION Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence: The record for Staff 1 contained a documented hire date of 06/12/2023. Staff 6 verified that Staff 1 began working at the center on 06/15/2023. The tuberculosis screening in the record for Staff 1 was dated 06/17/2023. The tuberculosis screening had not been completed prior to coming into contact with children.

All forms will be completed prior to coming into contact with children.

Description:
Based on observations, the center failed to ensure that children?s hands shall be washed with soap and running water or a disposable wipe before and after eating meals or snacks.
Evidence: The Toddler Class was observed finishing up breakfast, returning to their classroom and immediately beginning to play with various toys. The children did not wash their hands with soap and running water or a disposable wipe after eating breakfast.
Plan of Correction:
Will review the standards and remind all staff to wash children's before and after eating.
