Comments:
An announced initial inspection was conducted on 1/27/23 from 9:20am to 11:10am with the provider and one assistant. There were six children in care, ranging in age from two-years to four-years-old, for a total of 12 points. Children were observed playing with legos, puzzles, and practicing with lacing manipulatives. Six child records, the provider record, an assistant record, two household member records, and two pet records were reviewed. The provider has current certification in CPR and First Aid as well as Medication Administration Training (MAT). The provider plans to operate Monday through Friday from 7:00am to 5:30pm. The provider will care for up to 11 children from four-months to seven-years-old; with no more than five children in care under 30-months-old at any time. The home is equipped with a variety of age-appropriate materials. Meals are provided by the provider. Policies and procedures were reviewed. The first aid kit, flashlight, weather band radio, fire extinguisher, and smoke detectors were observed. A conditional, six-month license will be recommended. An unannounced follow up inspection will be conducted within 60 days to determine compliance with standards not observed at the initial inspection. Based on observation and interview, the provider did not ensure compliance with relevant state laws. Evidence: On 1/27/23 there were six children observed in care and the provider reported two other children are currently enrolled but were absent. State law requires a family day home to obtain a license prior to caring for more than four children.

Will reduce down to four until license is obtained.

Description:
Based on review of documentation, the family day home did not keep a written record of children in attendance each day. Evidence: On 1/27/23 there was no written record of children in attendance. The provider reported using an app to track attendance, but the app did not contain documentation of attendance for the day.

Will be sure attendance is maintained each day moving forward.
