Comments:
An unannounced monitoring inspection was conducted on 12/4/2023. There were 21 children, ages 2 months -5 years, 6 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records and 1 staff record were reviewed. The children were observed on the playground, during center time and circle time. The inspector arrived for the inspection at 9:20am and departed the center at 10:38am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on the review of four children?s records, the center failed to ensure that documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: Child #2 start date was 1/9/2023 and on the day of the inspection did not have the immunization record documented.

Plan of Correction: Provider will obtain immunization record.

Standard #: 8VAC20-780-520-C
Description: Based on observation, the center failed to ensure if diaper ointment or cream is used, these products shall be in the original container and labeled with the child?s name.

Evidence: Licensing Inspector observed in cubbies in the diaper changing area between the infant and toddler rooms Desitin, Aquaphor and Vaseline not labeled with the child?s name.

Plan of Correction: Provider will label diaper creams and ointment.
