Comments:
A complaint was received on April 19, 2024, and initiated on the same day. During the investigation interviews were conducted with staff on site and by phone, children and staff records were reviewed and classroom observations were made. On sight investigations took place on April 24 and May 10, 2024. The investigation concluded on July 3, 2024, with review of one child?s record. The information gathered supports the allegation therefore the complaint is valid. Violations regarding care and protection, supervision and diaper changing surface were observed and appear on the violation notice.
Based on observations and interviews the center failed to ensure the care and protection of children in care. Evidence: 1 - On April 18, 2024, during a diaper change Child #1 sustained an injury requiring medical attention including stitches to close a wound above the child?s left eye. 2 ? Staff #2 stated that while she was changing the diaper of Child #1, she looked away to attend to two other children and when she looked back Child #1 was moving around on the table and fell forward hitting the corner of his eye on the wooden window frame next the surface being used for diaper changing.

Based on observation the center failed to ensure that when one or more children are present for care one staff member and an immediately available staff member. Evidence: 1 ? On April 24 at approximately 1:20pm there was one staff present with 10 children in care. 2 ? Staff #1 stated that there was another staff member present, but she could not locate Staff #2 in the building. 3 ? Staff #2 stated that she did leave the building and had not communicated that to Staff #1.

Description:
Based on interview the center failed to ensure that the diaper changing surface used for diapering is a changing table or countertop designated for changing. Evidences: During the complaint investigation Staff #1 and #2 stated that the day of the incident the center was using a 10 stacked cots to change the diapers of children in care.
