8VAC20-790 Subsidy Program Vendor Requirements for Child Day Centers
Comments:
A subsidy health and safety inspection (SHSI) was conducted on April 5, 2023. There were no children present. A code compliance inspection was also conducted on this date. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.
Based on review, vendor did not ensure 2 of 2 staff have a national fingerprint check letter of eligibility prior to beginning employment.
Evidence: The records for staff #1 and staff #2 did not have a national fingerprint check letter of eligibility.
Plan of Correction:
Staff will circle back around and call the number provided to check on submission. Staff will check with staff that received central registry search results to see if these letters were received. Staff will re-submit if necessary.

Description:
Based on interview, vendor did not ensure there is at least one staff on duty who has obtained with the last three years instruction in performing a daily health observation of children.
Evidence: Staff #1 and Staff 2 stated they are not aware of either having completed daily health observation training. They are the only staff designated to work in the program at this time.

Staff will enroll in the class to get the certificate.

Description:
Based on observation, vendor did not ensure the required nonmedical emergency supplies are available.
Evidence: The radio was not operable during inspection.

A radio that works will be brought to the facility.
