Comments:
A code compliance inspection was initiated on March 26, 2024 and concluded on March 27, 2024. There was one school age child present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of three children?s records and three staff records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.
Based on review, facility did not ensure 1 of 3 staff documentation of required health information.
Evidence: The record for staff #2 (start date 6/25/2023) did not have documentation of being certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under supervision.
