Comments:
A subsidy health and safety inspection (SHSI) was initiated on March 26, 2024 and concluded on March 27, 2024. There was one child was present with two staff supervising. A code compliance inspection was also conducted on this date. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.
Based on interview, facility did not ensure there is a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: Staff reported they did not have documentation of child in attendance during inspection.

Evidence: The following was found during review of records for children currently enrolled: 1. The record for child #1 did not have parent's home information (address and phone number), employment information (place, address and phone number), emergency contact information (name, address and phone number) and a signed agreement with parent giving facility authorization to obtain emergency medical care if needed as well as agreement that facility will notify the parent if the child becomes ill and the parent agrees to have the child picked up as soon as possible.
