Comments:
An unannounced monitoring inspection was conducted on 1/19/2024. The inspector was on site from approximately 8:50 am-10:05 am. There were 6 children in care, ranging in age from 7 months to 12 years, with 2 caregivers supervising. Two of the children in care were household members. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 5 child records and 2 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.
Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-800-730-D
Description: Based on observation and interview, the provider did not ensure that household medications were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Three bottles of unlocked medication were observed on the kitchen counter.
2. The provider acknowledged they were household medications and were not locked.

Plan of Correction: medications have been placed in a locked cabinet

Standard #: 8VAC20-800-960-E
Description: Based on observation and interview the provider did not ensure that one infant's bottle was labeled with the child's full name and the date.

Evidence:
1. One bottle of formula was observed in the provider's refrigerator that was not labeled and dated.
2. The provider acknowledged that the bottle was not labeled and dated.

Plan of Correction: parent has been told to label the child's bottle

Standard #: 8VAC20-800-60-B
Description: Based on a review of five child records and interview, the provider did not ensure that one record contained the required information.

Evidence:
1. The record of child #1 (date of attendance: 1/18/2024) did not contain the name address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The record contained one of the two contacts.
2. The record did not contain the name and policy number of the child's medical insurance.
3. The provider acknowledged that the record was incomplete.

Plan of Correction: parent has been made aware of the missing info and will have it by the end of the week

Standard #: 8VAC20-800-90-A
Description: Based on a review of five child records and interview, the provider did not ensure to obtain documentation that one child has been adequately immunized according to the
requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of
Health regulations.

Evidence:
1. The record of child #1 (date of enrollment: 1/18/2024) did not contain an immunization record.
2. The provider acknowledged that the record had not been obtained.
