The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the department on June 4, 2024, regarding allegations of operating over licensed capacity. The inspection was initiated and completed on June 10, 2024. The inspector was on site from 9:40am to 12:15pm. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions regarding this inspection, please contact the Licensing Inspector at jessica.willis@doe.virginia.gov.
