Comments:
A mandated monitoring inspection was conducted on June 12, 2024 from 9:33 A.M.-1:30 P.M. There were 72 children present, ranging in ages from six to twelve, with ten staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation, the center failed to post the most current license in a conspicuous place.

Evidence:
The Conditional license that expired in 11/2023 was the license that was posted on the parent board. The renewed license was sent via email to the licensee on December 29, 2023.

Plan of Correction: Administration will locate the current license and ensure it gets posted.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to ensure that all food brought in from home was clearly dated and labeled in a way that identifies the owner.

Evidence:
In Classroom #23 there was one lunchbox that was not labeled with the name and date, and two lunchboxes that were not labeled with the date.

Plan of Correction: Administration will reinforce the importance of ensuring that all food brought in from home is labeled and dated. The director will do spot checks periodically to ensure that staff are in compliance.
