Comments:
A monitoring inspection was conducted on September 19, 2023 from approximately 8:50 am to 10:20 am to review voluntary registration requirements. There was two children present with one caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. A total of three children?s records and one caregiver record were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review, provider did not ensure 3 of 3 children?s records have documentation of required information.
Evidence: The following was found during child record review: 1. The record for child #1 (start date 5/21/2023) did not have parents? employment information and physical health examination record. 2. The record for child #2 (start state 5/15/2023) did not have parents? home information and parents? employment information. 3. The record for child #3 (start date 6/3/2023) did not have parents? employment phone numbers, emergency contact information, signed written agreement between parent and provider and physical health examination record.
Plan of Correction:
Provider will request information from parents and add to each child's record.

Description:
Based on observation and interview, provider did not ensure the criteria is met on the health and safety checklist at the time of the monitoring visit.
Evidence: The following was found: 1. The record for child #3 (start date 6/3/2023) did not have a medical release form from the family to permit emergency care. 2. The record for child #2 (start date 5/15/2023) did not have documentation of proof of identity. 3. The records for child #1 (start date 5/21/2023), child #2 (5/15/2023 and child #3 (6/3/2023) had a parent signed notification of liability insurance coverage however provider did not indicate on the forms whether the business is covered by liability insurance or not.
Plan of Correction:
Provider will request information from parents and it will be added to each child's record.
