Comments:
A renewal inspection was conducted on November 6, 2023 from 10:00 A.M.-12:00 P.M. There were eleven children present, ranging in ages from six to twelve years of age, with one staff and two volunteers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and one staff record, and three agent background checks were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

REPEAT VIOLATION Based on review of children records, the center failed to ensure that all required information was contained in the children's records.
Evidence: 1. Child #1's record did not contain the following; address for one of the designated people to call in an emergency if the parent cannot be reached, and the name home phone number, and home address of each parent who has custody. 2. Child #2 and #3's record only contained one of the two designated people to call in an emergency if the parent cannot be reached.
Plan of Correction:
Staff will follow-up with parents regarding their accounts and inquire about emergency contact information being provided prior to the child's first day of attendance.

Description:
Based on review of medication, the center failed to ensure that all medication was labeled with dosage amount.
Evidence: Child #1 had a medication, diphenhydramine, on site. The medication was not labeled with the dosage amount.
Plan of Correction:
Administration reached out to the parent to obtain the original container. In the future medication will not be accepted unless it is the original container and labeled properly.
