Comments:
An unannounced renewal inspection was conducted on November 15, 2023 from approximately 10:45 am to 11:55 am. Provider and assistant were present with nine children ranging in age from one year through preschool age. Children were observed participating in circle time and free play. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of five child records and two caregiver records were reviewed. Violations found are noted on the violation notice.
Repeat Violation Based on review, provider did not ensure 3 of 5 child records have documentation of required information.
Evidence: The following was found during record review: 1. The record for child #3 did not have start date, mother's employment phone number and medical insurance information. 2. The record for child #4 (start date 9/5/2023) did not have father's employment information and emergency contact addresses. 3. The record for child #5 did not have start date, father's home information, father's employment information and second emergency contact address.
Plan of Correction:
Parents will complete the information today.
