Comments:
An intial inspection was conducted on July 6, 2023 from approximately 1:30 pm to 2:45 pm. There were two children present with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 1 caregiver record and 5 household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review, provider did not ensure 1 of 6 household members has a completed central registry check.
Evidence: The record for household member #2 did not have a completed central registry check.
Plan of Correction:
Provider will reach out to the central registry unit to see if the previous request was received. Another central registry search was requested and submitted online.

Description:
Based on observation, provider did not ensure stationary outdoor playground equipment is placed at least six feet from the perimeter of other obstacles.
Evidence: There was not six feet around the perimeter of the outdoor stationary play set. The following measurements were taken: 2' from the rock wall to the hard border, 3.5' from the stairs to the hard border and 5' from the glider to the hard border.
Plan of Correction:
The outdoor play set will not be used until the border is extended to allow for 6' around the border and shredded rubber is added. A neighborhood playground is available within walking distance and children will be taken to this playground in the interim.
