Comments:
A renewal inspection was conducted on January 22, 2024 from approximately 10:10 am to 10:50 am. Provider and assistant were present with seven children. Children were observed playing freely and participating in table activities. Three child records were reviewed. A violation was found and is noted on the violation notice.
Repeat Violation Based on review, provider did not ensure 1 of 3 child records has documentation of required information.
Evidence: The record for child #1 (reported start date 12/11/2023) did not have documentation of start date, father's employment phone number, physician information and viewing proof of child's identity/age.
Plan of Correction:
Provider will request and add information to record today.
