Violations:
Standard #: 8VAC20-800-240-A
Description: Based on observations made, an area of the home was determined to be unsafe. Evidence: The rubber floor mats in the main daycare room were sticking up over the sliding glass door threshold approximately 1 1/2-2" and creating a trip hazard.

Plan of Correction: The provider said that she would make sure the rubber floor mats were adjusted/pulled back from the doorway so that there was not a trip hazard.
