Comments:
A monitoring inspection was conducted on 11/16/2023 from 9:45am-11:00am. An initial subsidy inspection was conducted on the same date. During the inspection there four children in care ranging in age from 23 months to 3 years of age. There was a total of 9 points in care with the provider. Children were observed eating breakfast and engaged in floor play. Children and provider records, emergency drills and emergency supplies were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed with the provider
Based on a review of children's records and interview, it was determined the provider did not ensure the record for each child contained all of the required information. Evidence: 1. The record for child #1 did not contain documentation of the child's enrollment date. 2. The record for child #2 did not contain documentation of the child's date of enrollment and an address for the emergency contact. 3. The record for child #3 did not contain documentation of the child's date of enrollment; the results of a health examination; the address of an emergency contact. 4. The record for child #4 did not contain documentation of the child's date of enrollment.
Plan of Correction:
The provider has document ed the children's enrollment dates. The parents of children with missing information have been contacted and will provide the address for emergency contacts and the results of a health examination.
