8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Comments:
An initial subsidy health and safety inspection (SHSI) was conducted on 11/16/23 from 9:45am to 11:00am. At the time of the inspection there were four children in care. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were observed, discussed with the vendor and appear on the violation notice..

Violations:
Standard #: 8VAC20-790-190-B
Description: Based on records review and interviews, it was determined that the vendor did not ensure that each child?s written or electronic record contains all required information.
Evidence:
1. The records for child #1, child #2, child #3, and child #4 did not contain documentation of proof of child?s identity.

Plan of Correction: Parents have been contacted and will provide their child's proof of identity to the vendor.

Standard #: 8VAC20-790-270-A
Description: Based on observation and interviews, it was determined that the vendor did not ensure that potentially poisonous substances, materials, and supplies such as, cleaning agents and disinfectants, shall be stored away in areas inaccessible to children.
Evidence:
1. There were cleaning products accessible to children in an unsecured lower cabinet under the kitchen sink.

Plan of Correction: The vendor stated she would move the cleaning supplies to a cabinet that is inaccessible children.

Standard #: 8VAC20-790-430-A-1
Description: Based on record review and interview it was determined the vendor did not ensure that emergency response drills were practiced at least monthly.
Evidence:
1. An emergency evacuation drill was not documented for October 2023.
2. The vendor confirmed the evacuation practice drill was not completed for October.

Plan of Correction: The vendor will ensure the emergency evacuation drill is conducted every month.
