Comments:
An unannounced monitoring inspection was conducted on April 25, 2024 between 8:30 am and 9:55am. There were 6 children present with two (2) caregivers supervising within the supervision guidelines. The children were observed arriving for the day, toileting, hand-washing, eating snack, and playing with other age-appropriate toys and materials. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas of administration, required posted documents, physical plant, staffing and supervision, programing, evacuation drill logs, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. There was no medication onsite at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review the provider did not obtain documentation of the physical examination within 30 days after the first day of attendance.
Evidence: The record for child #1 (DOE 2/5/24)did not contain documentation of a physical at the time of inspection. 2. The record for child #2 (DOE 3/16/24) did not contain documentation of a current physical at the time of inspection.

I will tell the parent today at pick-up to bring the physical within 2 weeks.

Evidence: The record for child #1 did not contain the work address for the parent and the address of one emergency contact.

Description:
Based on record review and interview the provider failed to obtain from each caregiver a tuberculosis screening dated within the last 30 days prior to date of hire.
Evidence: The documentation for TB test of caregiver #2 (DOH 4/01/24) was dated 9/01/23. The provider acknowledged the TB was older than 30 days prior to date of hire.

I will send her today to make appointment.
