Comments:
A initial inspection was conducted on June 6, 2023 from 10:20 am- 11:40am . The provider will have two living spaces designated for the children and outside area fenced in. The provider is requesting for license for 8 children ages ranging from 2-4 years old. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition.
Information gathered during the inspection determined area of non-compliance. Areas of non-complaince is noted on the violation notice.
Base on obstervation and interview, the applicant did not ensure a landline telephone was available , operable and accessible .
Evidence: Applicant confirmed appointment was schedule for a landline phone to be installed .
Plan of Correction:
The applicant had the phone installed on June 8, 2023 and contacted the LI when completed.
