Comments:
An unannounced renewal inspection was conducted on 7/21/2024 and 7/24/2023 for the Young Mens Christian Association of VA Blue Ridge - Woodrow. There were 61 children, ages 5 -12 years, 16 staff and 1 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications were observed during the inspection. A total of 5 children?s records and 5 staff record were reviewed. The children were observed during a restroom break, active time in the gymnasium and playground and lunch. The inspector arrived for the inspection at 9:50am and departed the center at 12:16pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review of five staff?s records, the center failed to ensure that all background checks were complete.

Evidence: The review of records for staff #3 showed no central registry results.
1. Staff #3 was hired on 1/16/2023 and on the day of the inspection did not have the required central registry results by the 30th day of employment documented.

Plan of Correction: Provider has obtained central registry results.

Standard #: 8VAC20-780-510-G
Description: Based on the review of five children?s records, the center failed to ensure that all medication shall be labeled with the child?s name, the name of the medication, the dosage amount and the time or times to be given.

Evidence: Medication for Child 3 was reviewed and on the day of the inspection did not include the child?s name, dosage amount and the time or times to be given.

Plan of Correction: Provider will obtain packaging for medication.

Standard #: 8VAC20-780-560-G
Description: Based on observation and discussion with staff, the center failed to ensure when food is brought from home the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence: Children?s lunch boxes were observed in the Beaver and Grizzlies classroom and were not labeled with the date and the child?s name.

Plan of Correction: Provider will label each child's lunch with name and date daily.
