Licensing Inspector Scott conducted an unannounced self-report inspection in response to allegations received by the licensing office on 12/18/2023, in the areas of abuse & neglect.
The investigation of the self-report was initiated on 12/19/2023 and findings were reviewed on 12/22/2023. As the information gathered during the investigation did support the allegations, the complaint is determined to be ?VALID.?

Based on staff interviews, the center failed to ensure that actions towards children that are forbidden by the standards (physical punishment, striking a child, roughly handling, or shaking a child) are not used at the center.
Evidence: 1.Staff 2 stated that she observed looking through the observation window of the classroom door, and witnessed Staff 1 standing over the child back. Staff 1 then pushed the child?s head into the carpet. Staff 1 then grabbed the child by both arms lifting the child to their face and scream ? DO NOT SPIT?.
2 Staff 2 stated they went into the class to talk to the staff member about what they witnessed. Staff 2 stated Staff 1 stated ?your right, I was frustrated, and he was spitting?.
Plan of Correction:
Staff member was terminated.
