Technical Assistance:
Discussed use of diapering surface, plastic bags, playground equipment and medication authorization.
Comments:
An unannounced mandated monitoring inspection was conducted on Decmber 19, 2023 from approximately 11:45 AM to 1:07 PM. Records were reviewed remotley for children. One caregiver was present with 3 children (6 points). Children were observed being picked up by parents and preparing for nap. Four children records and one medication records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, nutrition, children and caregiver records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings. Outdoor area was inspected.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review and staff interview, the licensee did not ensure to obtain documentation of a physical examination by or under the direction of a physician prior to child's attendance or within 30days after the first day of attendance.
Evidence: Child #3 start date was 07/07/2022, the provider did not have documentation of physical within 30days from first date of attendance.
Child #4 start date was 08/22/2023, the provider did not have documentation of physical within 30days from first date of attendance.
Plan of Correction:
Families were notified of physical required for attendance and were given 30 days to complete physicals.
