Discussed the following below:

8VAC20-800-830. Emergency response drills.
A. The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care
during all shifts that children are in care.
B. Shelter-in-place procedures shall be practiced a minimum of twice per year.

8VAC20-800-840. Injury records.
B. The information recorded shall include the following:
1. Date and time of injury;
2. Name of injured child
3. Type and circumstance of the injury;
4. Caregiver present and action taken;
5. Date and time when parents were notified;
6. Any future action to prevent recurrence of the injury;
7. Caregiver and parent signatures or two caregiver signatures; and
8. Documentation on how the parent was notified.


Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced mandated monitoring inspection was conducted on June 11, 2024 from approximately 11:00 AM to 12:05 PM. The inspection concluded on June 11, 2024. One caregiver was present with 4 children . Children were observed eating lunch , washing hands , freely playing and being picked up by parents. Four children records were reviewed . Licensing Inspector reviewed supervision, activities, equipment, nutrition, children and caregiver records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings. Outdoor area was inspected.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: REPEAT
Based on record review and staff interview , the licensee did not ensure a child's record contain all required information.

Evidence:

The licensee was unable locate child #3 's physical during the inspection. Child #3 was enrolled in 08/2023.

Record for child #4 did not have parent's signed acknowledgement of the receipt of the written information for parents, liability information and emergency response plan.
