Comments:
An announced initial inspection was conducted on 06/05/2023 from 1:35 p.m. to 2:15 p.m. The inspector reviewed compliance in the areas physical plant, programming, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the center failed to ensure that areas and equipment of the center were maintained in a clean, safe and operable manner. Evidence: 1. In the main classroom, there was one ceiling tile that was sagging, one ceiling tile that was missing, one vent missing a cover and one floor tile that was partially missing. 2. Staff #1 confirmed the ceiling tile sagging, another ceiling tile and the vent cover were missing, and the floor tile was partially missing.
