Comments:
A complaint was received and initiated on May 9, 2024 alleging a child sustained an injury while in the care of the center. The onsite investigation took place on May 14, 2024. During the investigation staff were interviewed at two center locations, children and staff records were reviewed and classroom observations were made. The information gathered during the investigation does not support the allegation and the complaint is not valid. However, violations were found that had not been reported. Those violations appear on the violation notice and were discussed with staff at the center and the owner at another center location at the conclusion of the onsite investigation.

Description: Based on observation the center failed to ensure that when staff are supervising children, they will always ensure their care, protection and guidance.
Evidence;
1 - Upon entering the center at approximately 11:30am Child #1, a 14 month old, was observed sitting in wheelchair in the infant care room a lone. There were not any adults in the room. There were not any additional children in the room.
a. The type of wheelchair Child #1 was sitting in required another person to maneuver the chair around. The child could not self propel the chair.
2 - Additionally, there was a large changing table blocking the doorway from the infant room leading into the two year old room. This doorway was the only entry to and from the infant room from inside the building.

Plan of Correction: Due to the connecting classrooms, the teacher was about to change another child and stepped into the other classroom to move the changing table. Teachers will stay in their assigned classrooms at all times.

Description: Based on observation the center failed to maintain required ratio.
Evidence:
Upon arrival there was one 14 month old child alone in the infant classroom without any staff member being present in the room.

Plan of Correction: Staff will estate in classroom while floater does lunches While doing lunches, a floater will heat lunches so that Teacher stay in the assigned classrooms .

Description: Based on observation the center failed to ensure that when children are in ongoing mixed age groups, the staff to children ratio applicable to the youngest child in the group shall apply to the entire group
Evidence:
1 - At approximately 11:30am there six children in the designated two year old with one staff. However, the children present in the room ranged in age from 17months through two years, eight months old. Therefore, an additional staff person was required.
2 - Staff stated that the additional staff member was had stepped out to make lunch.

Plan of Correction: When going to heat lunches, there will be a floater to assist with lunches so that teachers stay in their assigned classrooms at all times .
