Comments:
An unannounced monitoring inspection was begun on 2/29/24 in response to a self reported incident being received in our office on 2/27/24 concerning care of children.

Multiple interviews were conducted with a parent, staff and administration. The preponderance of evidence supports the allegation.

Violations:
Standard #: 8VAC20-780-400-B
Description: Based on multiple staff statements, behavioral guidance was not constructive in nature.
Evidence:
1. On multiple occasions, different children in the Children's House rooms have been forced to stay on their cots during rest time.
2. Staff have placed their hands on children's backs and applied pressure, placed their arm across the children's backs and held onto the opposite side of the cot or placed their ankles/legs across the children to keep them on the cot.
3. Staff # 1, 2, 3 and 4 confirmed this had occurred.
4. Child A expressed fear about naptime at the center due to this practice.

Plan of Correction: Corrective action has included initial self reporting of this incident to CPS
and Licensing, as well as an internal investigation. Parent communication via email, phone and in person has occurred. All staff regardless of involvement,
has been required to complete training in Safe Sleep Practices, Manadated Reporting and Training and Professional Development The Virginia Health and Safety Update Training, review all licensing standards for rest periods and discipline, and additional best practices for childcare workers as well as Guidepost Montessori best practices as methods used do not align
with our values. Training and support will be ongoing. Individual disciplinary action has occurred. Ongoing corrective action began on 2/27/24 when incident was first
reported. Steps in this process will be ongoing in relation to training and staff support to ensure that these practices are never repeated. All families have been talked with personally to help answer questions and address concerns.
