Comments:
An unannounced inspection was conducted today between 12:40pm and 1:15pm in response to a complaint received by the Fairfax Licensing Office in regards to daily activities for children and child supervision guidelines.
During the inspection the substitute provider was supervising 7 children (including two resident children who do not count in the capacity). Based on the ages of the children present it is determined that an additional caregiver was needed to meet the supervision guidelines. The complaint in therefore deemed to be 'valid". The complaint related violation and other non related violations can be found on the violation notice.

Based on record review, caregiver records did not contain all required information. Evidence: Caregiver 2's record did not contain documentation of at least two references, the name and contact information for a person to contact in an emergency, or documentation of the caregiver qualifications.

Evidence: Caregiver 2 was operating in a substitute provider role on the date of inspection. There was not a time of arrival for Caregiver 2 in that position for the day.

COMPLAINT RELATED Based on observation and record review, the provider failed to ensure that a caregiver does not exceed 16 points by using the following point system: 1. Children from birth through 15 months of age count as four points each; 2. Children from 16 months through 23 months of age count as three points each; 3. Children from two through four years of age count as two points each; 4. Children from five years through nine years of age count as one point each; and 5. Children who are 10 years of age and older count as zero points. Evidence: At the beginning of the inspection there were 7 children present with one caregiver. The point total for the 8 children was 25 points, 9 points over the supervision guideline.
Plan of Correction:
Moving forward will be very careful not to surpass point system when needing to leave caregiver as my substitute.
