Comments:
A renewal inspection was conducted on January 11, 2024 from 10:20 A.M.-12:45 P.M. There were 33 children present, ranging in ages from 2 years 6 months to five years of age, with ten staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, five staff records, and two agent background checks were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of staff records, the center failed to ensure that tuberculosis documentation shall been completed within the last 30calendar days of the date of employment.

Evidence:
1. Staff #2's date of hire was 08/23/2023. The tuberculosis documentation on file was dated 07/28/2022.
2. Administration verified the date of employment and the date on the tuberculosis documentation.

Plan of Correction: Staff have made an appointment with their doctor. It should be completed by 01/25/2024.

Standard #: 8VAC20-780-40-E
Description: Based on review of medications and the medication policy, the licensee failed to ensure compliance with the center's own policies and procedures.

Evidence:
1. In Classroom #1 there was an emergency prescription medication, (Epi-Pen), that expired in December 2023.
2. Staff#1 verified the expiration date on the prescription box and Epi-Pen.
3. In the 2023-2024 Parent Handbook it states under Medication Administration that "all medication at the school shall be kept on file, and the director shall conduct a monthly verification of medication to ensure medication has not expired. Expired medication shall be returned to the family."

Plan of Correction: The teacher is waiting for the parent to bring in a new Epi-Pen that is not expired before returning the one that is at school. Child can not return to school after 1/24/2024 unless there is a new Epi-Pen.

Standard #: 8VAC20-780-90--A
Description: REPEAT

Based on review of children records and interview, the center failed to ensure that a written agreement between the parent and center to address authorization to seek emergency care, pick up if a child becomes ill, and notification if someone in the child's household contracts a reportable communicable disease was in each child's record by the first day of attendance.

Evidence:
1. Child #4's date of enrollment was 01/02/2024. The agreement in the record was signed by the parent on 01/03/2024.
2. Child #5's date of enrollment was 01/02/2024. The agreement in the record was signed by the parent on 01/08/2024.

Plan of Correction: Before the first day of enrollment Director will make sure to have all agreements and paperwork corrected.
