Technical Assistance:
Documentation of viewing proof of identity for children is also required to be on record, but is not able to be cited at this time. Moving forward, documentation should be maintained of meeting this requirement as well.

Comments:
An unannounced Code Compliance inspection was conducted with the Director of the religious-exempt program on 10/31/23 from 11:20am to 12:45pm. There were 37 children in care, ranging in age from six-months to four-years-old, supervised by eight staff. Children were observed eating lunch brought from home, playing on the playground, and washing hands. The required postings, classrooms, and playground were observed. Five child and five staff records were reviewed for required documentation. The program met the requirement for staff on-site with current CPR/First Aid training and Daily Health Observation training. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 11/14/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-770-40-D-2
Description: Based on review of five staff records, the program did not obtain a sworn disclosure statement prior to employment for one staff, or documentation of the results of a search of the central registry for one staff within 30 days of employment. Evidence: 1. The record for Staff #3, with an employment date of July 2023, did not contain a sworn disclosure statement. 2. The record for Staff #4, with an employment date of 8/10/23, did not contain documentation of the results of a search of the central registry.

Plan of Correction: Staff #3 signed sworn disclosure statement on site during inspector's visit, 10/31/23. Moving forward, prior to hire, staff will complete sworn statement before employment begins.

Staff #4 CRS was in portal awaiting portability. Document was cleared and placed in staff's folder on 11/2/23. Moving forward, the documents shown as "pending" will be printed and placed in staff's folder.
