Comments:
An unannounced monitoring inspection was initiated and concluded on Thursday, April 25, 2024. The licensing inspector was onsite from 10:00am until approximately 11:30am. There were four children in care and two caregivers present. The inspector reviewed compliance in the areas of administration, physical plant, and programming. During the inspection, four children's records, two caregivers' records, two household members' records, and one household pet records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of four children's records and interviews, the provider did not meet the criteria on the health and safety checklist at the time of the monitoring visit.
Evidence: 1) The record of Child #1 (DOA: 04/02/24) did not contain a signed acknowledgement that the family day home business is covered by liability insurance. During interview, Caregiver #1 reported the form was given to the child's parent, but the signed form acknowledging receipt had not been returned.
2) The immunizations in the record of Child #1 (DOA: 04/02/24) were dated 04/17/24, after the child's admission to the program. 3) The immunizations in the record of Child #4 (DOA: 03/18/24) were dated 04/16/24, after the child's admission to the program. Proof of adequate immunizations should be received on the date of admission or prior to admission.
Plan of Correction:
The signed form will be obtained on 04/26/24.
In the future, immunizations will be obtained prior to children's admission to the program.
