Comments:
An unannounced monitoring inspection was conducted on October 26, 2023, from 4:00 p.m. through 5:00 p.m. Upon arrival, there were 23 children, ages 5 through 10, in the care of 3 staff.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records were reviewed. Annual review of staff records will be scheduled at a later date,
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review the center failed to have documentation of proof of identity for one child.
Evidence:
1. Five children records were reviewed. 2. Child #4 did not have documentation of the center viewing proof of identity. 3. Staff confirmed there was no documentation of reviewing proof of identity for Child #4.
