Comments:
Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on September 25, 2023. There were 8 children ages 17 months to 4 years old present under the direct care of 2 staff. The Licensing Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of documentation, the Center failed to disclose in writing to the parents or guardians of the children in the center the qualification of the personnel employed.
EVIDENCE: Center did not disclose "in writing" the personnel qualifications to the parents/guardians.
Plan of Correction:
Center Director will update the parent handbook to include staff qualifications.
