Comments:
An unannounced complaint inspection was conducted on 02/08/2024 in response to allegations received by the licensing office on 01/30/2024 in the areas of staff qualifications and transportation. There were 55 children, ages 16 months -4 years, and 13 staff providing direct care and supervision to children present during the inspection. Staff were interviewed and a total of 3 staff records were reviewed. Two inspectors arrived for the inspection at approximately 8:25 am and departed approximately 10:10 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider . The information gathered pursuant to an investigation of the complaint supports the allegation and the complaint was determined to be valid.
COMPLAINT RELATED REPEAT VIOLATION Based on record review, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence 1. Staff 4 confirmed the hire date for Staff 1 was 02/01/2024. No eligibility letter was available for review. Staff 1 was observed working at the center on 2/8/2024. 2. Staff 2 had a documented hire date of 02/05/2024. No eligibility letter was available for review. Staff 2 was observed working at the center on 2/8/2024.
