Comments:
An announced initial inspection was conducted on 10/04/2023. At the time of entrance, no children were in care with the provider. The record sample size consisted of sample children's record and the provider's record and the household member's record. There were no children in care so the requirements related to childcare could not be observed at his time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.
Based on observation, record review, and interview it was determined the provider if not meet the criteria on the health and safety checklist at the time of the initial evaluation. Evidence: 1. The provider did not have proof of rabies vaccination for the two dogs on the property. 2. The required emergency phone numbers were not posted. 3. Provider confirmed the above were not available during this inspection.
Plan of Correction:
Provider got rabies vaccinations for both dogs and posted emergency numbers by the phone.
