Comments:
An unannounced 2 month monitoring inspection was conducted on November 29, 2023, from 9:25am-10:40am. At the time of entrance, ( 1 ) child ( 2 ) points was in care with (1 ) caregiver present. The inspector reviewed 1 child?s record, the caregiver and household records are current. The child was observed playing with toys and building with magnetic shapes. No medications were on site. Areas of non-compliance are identified on the violation notice. Several questions about the standards were discussed with the provider.

Based on a review of child's record, the record does not contain all the required information such as medical insurance policy number. Evidence: Child #1, date of first attendance 10/30/2023 does not have documentation of the medical insurance policy number

The provider will ask the parent for the medical insurance policy number.

Description:
Based on a review of 1 child's record, the parents have not been provided in writing the policies for administration of medication. Evidence: Child #1, first day of attendance 10/30/2023 does not have on record the policies for administration of medication before the child's first day of attendance.
Plan of Correction:
The provider will provide the form to the parent for signature and file in the child's record.
