An unannounced initial inspection was conducted on 11/1/2023 from 10:34 am-11:40 am. There were 3 children supervised by 1 direct caregiver. The home, 1 caregiver record, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Based on record review and interview, the center provider failed to obtain an out-of-state child abuse and neglect search request and criminal history name check for household members and caregivers.
Evidence: 1. The record of caregiver #1 did not contain documentation of an out-of-state criminal history check. 2. Caregiver #1 indicated living in another state other than Virginia within the last five years on their sworn disclosure statement dated 09/04/23. 3. The record of household member #1 did not contain documentation of an out-of-state central registry check and criminal history check. 4. Household member #1 indicated living in two states other than Virginia within the last five years on their sworn disclosure statement dated 09/22/2023. The record of household member #2 did not contain documentation of an out-of-state criminal history check. 4. Household member #2 indicated living in two states other than Virginia within the last five years on their sworn disclosure statement dated 09/22/2023.
