Comments:
An unannounced renewal inspection was conducted on May 7th, 2024. Two licensing staff were on site from approximately 9:25am until 11:50am. There were six children present with one staff supervising. There was a total of two classrooms visited. The age range of the children were from infant to toddler. Children were observed completing an art activity, playing with books, listening to educational music. The licensing staff interacted with staff and children in care.

The two licensing staff reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please reach out to the licensing inspector.

Standard #: 8VAC20-780-70
Description: Based on record review, staff records did not contain all required documentation.
Evidence:
1. Staff #2 (DOH 04/24/2024) did not have documentation of completed references in the file. Director confirmed the references were completed, but not documented.
2. Staff #2 (DOH 04/24/2024) records did not contain documentation of the completed orientation training. The director acknowledged the document was completed but not at the center at the time of the inspection.

Description: Based on record review, the center failed to obtain a signed agreement between the parent and the center by the first day of the child?s attendance.
Evidence:
1. The written agreement for child #1 (date of attendance 04/22/2024) was dated and signed by a parent after the first day of attendance on 04/24/2024.

Description: Based on observation and interview, the center did not ensure during the stated hours of operation, there shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff, volunteer, or other employee.

1. Upon arrival at approximately 9:25am, the director was the only staff present with six children in the building.

2. Director acknowledged that they were the only person present with the children in the building when licensing arrived and stated that the second staff person stepped out and would be returning. At approximately 10:10am, the director reported that the additional staff was not returning to the center that day.

Description: Based on observation, the center did not ensure children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
1. Classroom A contained five children and one staff supervising when licensing staff arrived.
2. During the walkthrough of the building five minutes after licensing staff arrived, classroom B was observed having one sleeping infant without a staff member present.
3. Director acknowledged that the child was waking from their nap and was located away
from Classroom A out of sight of the staff.
4. No other staff were present in the building at the time of the inspection.

Description: Based on observation and interview, the required ratio of one staff member to four children was not maintained.
Evidence:
1. On 5/07/2024, two licensing staff observed five children in Classroom A with one staff member. The youngest child was five months old. One additional staff member was required.
2. On 5/07/2024, two licensing staff observed one child (15 months) in Classroom B unsupervised. One staff member was required.
3. The director acknowledged that the center was out of ratio.
