Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced renewal inspection was initiated on June 12, 2024 from approximately 10:25 am to 12:10 pm. There was a total of 19 children with 5 staff. The children were observed during outdoor play and napping. A total of 5 children records and 6 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on records review, the center did not ensure each child has a physical examination by or under the direction of a physician before or within 30 days after the first day of attendance.
Evidence:
Child #5 (start date: 2/20/24) did not have a physical examination by or under the direction of a physician before or within 30 days after the first day of attendance. One physical examination was dated 2/20/23 and one was dated 4/15/24.

Plan of Correction: Mom notified the school that a physical including immunization records will be submitted before the child comes back from vacation

Description: Based on records review, documentation of references, when a reference check was taken over the phone, did not include all information.
Evidence:
1. 2 out of 2 references for Staff #1's (start date: 03/19/2024) reference check did not include the dates of contact.
2. 1 out of 2 references for Staff #6's (start date: 02/14/2024) reference check did not include the dates of contact.

Description: Based on records review, the center did not ensure staff complete orientation training in first aid and cardiopulmonary resuscitation (CPR) within 30 days of the first day of employment.
Evidence:
Staff #2 (start date: 3/25/24) and Staff #4 (start date: 3/27/24) did not complete orientation training in first aid and CPR within 30 days of the first day of employment.

Plan of Correction: Orientation will be completed within 24 hours after submitting this report

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside, were maintained in a safe condition.
Evidence: There was chipped paint on a wall and chipped wood on the bathroom door frame in the Preschool 4 classroom.

Plan of Correction: Chipped paint and wood will be fixed by this weekend

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure hazardous substances, such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children.
Evidence: Air freshener, sanitizing spray and disinfectant solution were located in an unlocked cabinet above the refrigerator in the following classrooms: Infant A, Infant B, Preschool 3 and Preschool 4.

Standard #: 8VAC20-780-510-N
Description: Based on record review, the center did not keep a record of medication given to children that included all information.
Evidence:
1. Child #7 had a prescription medication that was administered by the center on May 30, 2024 and May 31, 2024.
2. The medication record did not include the name of the staff member who administered the medication.

Plan of Correction: An approved paper version of the medication log will be used instead of electronic

Description: Based on observation, the center did not ensure that over-the-counter skin products are not kept beyond the expiration date of the product.

Evidence: Child #6?s diaper cream expired in February 2024.

Plan of Correction: Expired cream was returned to parent

Standard #: 8VAC20-780-530-A
Description: Based on observation and record review, the center did not ensure that at least one staff in each classroom or area where children are present have current certification in first aid.
Evidence:
1. Staff #2 was observed working alone in the Infant A classroom. Staff #2 does not have current certification in first aid.
2. Staff #4 was observed working alone with the Preschool-4 classroom. Staff #4 does not have current certification in first aid.

Plan of Correction: trainer was contacted to correct certification scheduled for 07/08/2024
