Comments:
An unannounced non-mandated monitoring inspection was conducted on June 11, 2024 between 9:50am and 11:02am. There were 3 children present with two (2) caregivers supervising within the supervision guidelines. The children were observed playing with various manipulatives, coloring, diapering, toileting procedures, and handwashing. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas of administration, required posted documents, physical plant, staffing and supervision, attendance, child, household members and caregiver records. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the provider did not obtain documentation that the child had been adequately immunized according to the requirements of the Code of Virginia and applicable State Board of Health regulations. Evidence: 1. The file for Child #1 (DOE 6/6/24) did not have certified immunization documentation by a licensed physician or health official. The computer print-out of immunizations was dated 1/4/23. 2. The file for Child #2 (DOE 6/6/24) did not have certified immunization documentation by a licensed physican or health official. The computer print-out of immunizations was dated 3/20/23.
Plan of Correction:
I will contact the parents today that they must bring in proof of their child's immunization record. It must be certified by a licensed physician or other health official.
