Comments:
An unannounced inspection was initiated on 5/16/2024 in response to a complaint that was received by the licensing department on 5/16/2024 regarding allegations in the areas of supervision, building maintenance, injury reporting, behavioral guidance and forbidden actions.
The inspection was initiated on 5/16/2024 and completed on 5/28/2024. On 5/16/2024, the inspector was on site from approximately 12:31 pm to 3:20 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted remotely.

The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Standard #: 8VAC20-780-400-B
Complaint related: Yes
Description: Based on interview, the center did not ensure that behavioral guidance was constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Evidence:
1. Staff #1 (date of employment: 2/23/2024) stated that she used her leg to keep child #1 on her cot for approximately two minutes. She reported that child #1 did not want to go to sleep so she used her leg to keep her on her cot so she would settle her body and take a nap. Staff #1 acknowledged that she was not trained to get children to sleep in that manner.
2. Administration acknowledged that staff #1's actions were not appropriate.

Plan of Correction: Behavior guidance along with ages ad stages appropriate behaviors are part of our orientation. Staff #1 was re-coached on behavior guidance tools.

Description: Based on interview, the center did not ensure that the action of forcing rest was forbidden.

Evidence:
1. In interview, staff reported that child #1 (age 22 months) was forced to lay down on her cot by staff #1 (date of employment: 2/23/2024). In interview, staff #1 acknowledged that she placed one leg over child #1's cot to keep her laying on her cot so she would go to sleep. She stated that when another staff member told her to stop, she stopped immediately. It was estimated that her leg was placed over the child's cot without pressure for approximately two minutes.
2. Administration acknowledged that attempting to force rest was forbidden.

Plan of Correction: Napping strategies are part of orientation. Staff #1 was coached on proper napping techniques.

Description: Based on interview, the center did not ensure that parents were notified by the end of the day of minor injuries as required.

Evidence:
1. In interview, it was reported that child #2 (age 12 months) sustained a minor injury on his finger which was not reported to the child's parents. In interview, staff #2 stated that she applied first aid to the child's finger in the form of an ice pack.
2. Administration were not able to produce documentation that the parent of child #2 was notified of the injury the day of occurrence.

Plan of Correction: Starting immediately all accident/injury forms are to be completed then loaded into Kaymbu (our communication app) to give immediate notification and documentation.

Complaint related: Yes
Description: Based on a review of documentation and interview, the center did not ensure to maintain a written record of children's minor injuries in which entries are made the day of occurrence.

Evidence:
1. Staff reported that child #2 sustained a minor finger injury. The center was unable to locate documentation of a written record of the injury that included the required information.
2. Administration acknowledged that an injury report for the incident could not be located.

Plan of Correction: Starting immediately all accident/injury forms are to be completed then loaded into Kaymbu (our communication app) to give immediate notification and documentation.
