Technical Assistance:
Technical assistance was provided regarding record keeping, menu requirements when providing food, and storage of potentially poisonous substances.
Comments:
An announced initial inspection was conducted on 7/10//2024 between the hours of 9:45am-11:45am. There were two children in care supervised by the provider. Children were observed having morning snack and playing freely. The inspector verified the manufacture date for one crib. A variety of age appropriate materials were observed in the family day home. The inspector reviewed 1 caregiver record, 3 household member records, 2 children's records, and the Emergency Preparedness Plan submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined violations with applicable standards or law. Areas of non-compliance are identified in the Violation Notice. Based on record review, a sworn statement was not obtained for 1 caregiver and 2 household members.
Evidence: 1. The record for Caregiver #1 did not contain documentation of a sworn statement. 2. The records for Household Member #1 and Household Member #2 did not contain documentation of a sworn statement.

Sworn statements will be completed and evidence of completion will be provided to the department within 10 days.

Description:
Based on observation, potentially poisonous substances such as disinfectants, were not stored in areas inaccessible to children.
Evidence: 1. Two cans of disinfectant spray were observed in the bathroom accessible to children.
Plan of Correction:
The disinfectant sprays were removed during inspection.
