Comments:
An announced initial inspection was conducted on June 4, 2024, from 9:15 am until 11:30 am. Additional documentation was obtained on June 11, 2024. The program director was present during the inspection. The program plans to provide care for children two months through five years in age. The program will operate Monday through Friday from 7:00 am to 6:00 pm. The facility can accommodate the requested capacity of 54 children. The facility consists of four classrooms, a multipurpose room and outdoor play space.
The center is equipped with toys, educational materials, games, furnishing, arts and crafts materials and other items for the children to use. All areas of the facility were inspected.
Policy and procedures were submitted to the department and reviewed. Transportation will not be provided. Medication will be administered with proper parent authorization when needed. The center will provide meals.
During the inspection, the inspector reviewed compliance in the areas of administration, physical plant, staffing, medication, special care and emergencies and nutrition. Compliance with some standards could not be determined since the center is not yet in operation. A total of nine staff
records were reviewed along with background checks for the board officers.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
The center is in substantial compliance with the standards that could be determined. A six-month
conditional license will be recommended. During the conditional period, the center will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations:
Standard #: 22.1-289.035-A
Description: Based on review of nine staff records and interview, the center did not ensure one staff
obtained a repeat background check every five years.

Evidence:
1. The record for staff #3, employed 10/20/2016, contained the results of a central registry check dated 03/06/2019, exceeding the requirement to renew every five years.
2. The director stated a request to update the central registry had been submitted on 02/07/2024, however the results were not received and a follow-up to the status was not conducted until 05/17/2024.

Plan of Correction: Per the director:
A new plan for tracking background checks and follow-up has been initiated with the new YWCA HR director that started 03/08/2024.

Standard #: 22.1-289.058
Description: Based on observation and interview, the center was not equipped with a carbon monoxide
detector.

Evidence:
The director stated they did not have a carbon monoxide detector installed in the center.

Plan of Correction: Per the director: A carbon monoxide detector has been installed.

Standard #: 8VAC20-780-540-E
Description: Based on observation and interview, the center did not maintain one working, battery operated
radio in each building used by children.

Evidence:
The center had a radio, however, the batteries in the radio did not work. The
director stated she did not have additional batteries on site.

Plan of Correction: Per the director: Batteries have been purchased. There is a working radio with back-up batteries.
