Comments:
An unannounced, on-site monitoring inspection was initiated on 7/10/2024 and completed on 7/10/2024, as a part of the conditional licensure period. The on-site inspection began at 10:15AM and ended at 11:48AM. The inspector reviewed compliance in the areas listed above. There were 41 children, 6 staff present. The inspector reviewed 4 children?s records, 2 staff records and 6 medications on-site. This inspection included-
? document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), ? tour of the facility, ? interviews, ? observations (this would include observations of photos, recordings, etc.)
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.
Hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. Licensing inspector observed 2 bottles of cleaning solution within reach of school age children.

Provider will remove cleaning solution from reach of children.

Description:
When food is brought from home the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Licensing inspector observed 9 lunch containers that were not labeled with a name and date to identify the owner.
Plan of Correction:
Provider will date and label lunches with names.
