Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on July 21, 2020 and concluded on July 21, 2020. The director was contacted by telephone to initiate the inspection. There were 14 children present and five staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on review of two children's records, the center did not obtain all required documentation for one child's record.
Evidence:
1. The record of child #1, enrolled 02/10/2020, did not contain the house number of the second emergency contact. 2. The director acknowledged that the second emergency contact for child #1 did not have a house number listed.
Plan of Correction:
Per the Director: I will contact the parent now and obtain the missing information.
