Comments:
An unannounced monitoring inspection was conducted on Monday, October 23, 2023. The inspection began at 10:00am and concluded at 11:45am. There were 26 children present, ranging in ages from 2 years to 4 years, with 5 staff supervising. Children were observed playing with puzzles and other age appropriate toys, participating in circle time, and handwashing. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Repeat Violation - Based on record review and interview, the center failed to ensure that all employees shall undergo background checks every five years. Evidence: 1. The staff record for Staff #3 contained a national criminal background check dated 6/27/2018. Staff #2 should have undergone a criminal background checks on or before 6/27/2023.
