Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 12/16/2021 and concluded on 12/21/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 13 children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-70
Description: Based on a review of three staff records and interview, the center did not ensure that one staff record contained all the required information.

Evidence: 1) The licensing inspector reviewed the record for Staff #3, hired on 11/03/14. The record did not contain the staff member's most recent central registry finding. 2) During interviews, a member of management reported the central registry for Staff #3 was received on 04/12/18, but the original document was misplaced and not maintained as required. A central registry finding is a background check required by the regulation entitled Background Checks for Licensed Child Day Centers and required to be in each staff record.

Plan of Correction: The provider contacted the Office of Background Investigations and requested the central registry findings for Staff #3. In the future, all background checks will be maintained in staff records as required.
